This research was targeted to study, analyze, design, and develop an information system for diagnosis and treatment service of Oral and Maxillofacial surgery. The Oral and Maxillofacial Surgery Unit of The Dental Hospital University of Chulalongkorn University was the case study. The researcher designed and developed an information system for diagnosis and treatment service of Oral and Maxillofacial surgery in which covered diagnosis, treatment planning, medical treatment, surgical treatment, operating room booking, hospital room booking, surgical instrument booking, follow-up, patient appointment and anesthesia. The system was designed based on user's requirements of both message and image information for user's convenience. Including monitor and control secure the system to prevent unauthorized access. The researcher employed Microsoft Visual Basic 6, the system development application, to manage relational database of Microsoft SQL Server 2000 as the database management system, and employed Crystal Report 8.5 as a report generating application for the system. The tools were operated with Microsoft Windows 2000. In addition, the researcher generated 'Trigger' in order to store any alteration generated to trail if there was any doubt. The system operated under a multi-user environment. After the system developed, the researcher together with dentists from the Oral and Maxillofacial Surgery Unit of The Dental Hospital University of Chulalongkorn University test drive with the actual data. The system was proved to operate accurately and accordingly to user's requirements. This research was to be beneficial to the Oral and Maxillofacial Surgery Unit to use diagnose and treat service of oral and maxillofacial surgery and support user for further research.
