Medication errors during hospitalization can lead to adverse drug events, harming at least 1.5 million people every year and extra medical costs about $3.5 billion a year. Accurately and completely reconcile medication could reduce medication errors and complete medication reconciliation can develop by hospital pharmacist. The purpose of this study was to survey and analyses situation of medication reconciliation in Thailand especially attitude and understanding of hospital pharmacist. This is a mixed quantitative and qualitative evaluation study. In the quantitative evaluation was cross-sectional survey by using mail questionnaires. The sample consisted of 1,376 hospitals. The response rate was 42.12% (567 Hospitals). In the qualitative evaluation were in-depth interview and observation. The sample consisted of 7 hospital pharmacists. Data were collected during November 2008- February 2009 This study found that situation medication reconciliation process score mean±SD= 51.30±2.5%, pharmacists’ participation score mean±SD= 57.89±3.11%. Attitude of hospital pharmacist, pharmacist should participate in medication reconciliation process. The relative importance barriers encountered during the implementation of the medication reconciliation program were, Lack of team work among disciplines, extra burden and workload. Hospital pharmacists have a good understanding in medication reconciliation. Believe in medication reconciliation can reduce medication error and good attitude on medication reconciliation. The success factors were teamwork among disciplines and support from hospital.
