การบริบาลทางเภสัชกรรมสำหรับผู้ป่วย ในหอศัลยกรรมทั่วไปชายที่โรงพยาบาลมหาราชนครราชสีมา / พรชัย ปุ่นโพธิ์ = Pharmaceutical care for patients in male-surgical ward at Maharat Nakhornratchasima Hospital / Pornchai Poonpho
Examines the outcome of pharmaceutical care services for surgical patients in the following areas : numbers and category of drug therapy problems (DTPs) that were found and were resolved or prevented. The study was done at fifth-right floor male-surgical ward at Maharat Nakhonratchasima Hospital during December 2002 to February 2003. The pharmacist performed the pharmaceutical care services by interviewed the patients to collect the medication history, monitored drug therapy process, and adverse drug reaction outcome, counseled the patients about drug usage, teamed up and coordinated with other health professions to resolve or to prevent the DTPs, and finally followed up on the suggestion. Two hundred twenty-seven patients were serviced by the pharmacist. Most of the patients were 41 years old and over (64.8%). The mean drug used was 6 items for each patient. Sixty-six DTPs were detected from 58 patients (25.1%). The pharmacist made suggestions to resolve 33 DTPs, however only 26 of them were resolved. The numbers and category of DTPs found and resolved were as followed: 1) patients needed additional drug therapy, 23 problems detected, 9 resolved. 2) patients were taking unnecessary drug, 4 problems detected. 3) patients were taking wrong drugs, 2 problems detected, 1 resolved. 4) patients were taking too little of the correct drug, 21 problems detected, 6 resolved. 5) patients were experiencing on adverse drug reaction, 4 problems detected, 2 resolved. 6) patients were taking too high of correct drug, 10 problems detected, 6 resolved. 7) patients were noncompliance 2 problems detected, 2 resolved. The study sggested that in the pharmaceutical care service provided by pharmacist plays an important role in drug therapy monitoring by identifying, preventing and solving drug therapy problems. It will prevent and decrease the adverse drug reactions that might occur. This in turn improves efficacy in the patient care services
