การประเมินผลการบริบาลทางเภสัชกรรมในผู้ป่วยโรคความดันโลหิตสูงที่ศูนย์สุขภาพชุมชนอุตรดิตถ์ / สุภาภรณ์ เจตะบุตร = Evaluation of pharmaceutical care in hypertensive patients at Uttaradit Primary care Unit / Supaporn Chetaboot
The purpose of this experimental design study was to assess the outcomes of pharmaceutical care in hypertensive patients at Uttaradit Primary Care Unit (PCU) in three categories: drug therapy problems (DTPs), therapy compliance and clinical outcomes of patients. The hypertensive patients were divided into 40 pairs of the control and study groups by matching method according to their sex and ages. After the first assessment, the study group were provided pharmaceutical care at PCU and at home whereas the control group received the traditional care at PCU. The second assessment in both groups were performed at two months later. The results showed that there was a decrease of 56.7% in DTPs in the study group (from 120 to 52 problems) and 17.6% in the control group (from 91 to 75 problems). The therapy noncompliance in the study group was decreased 43.6% (from 55 to 31 problems) and 13.0% in the control group (from 46 to 40 problems). Systolic/diastolic blood pressure in the study group was reduced from 139.2 +- 3.1/80.2 +- 1.4 to 131.0 +- 2.8/78.0 +- 1.2 mmHg and reduced from 137.5 +- 3.0/81.2 +- 1.5 to 134.0 +- 2.6/77.2 +- 1.5 mmHg in the control group. The number of DTPs and therapy noncompliance between both groups were significantly different (p=0.000 and p=0.003, respectively). However, the systolic and diastolic blood pressure between both groups were not significantly different (p=0.292 and p=0.631, respectively). Therefore, pharmaceutical care can reduce the number of DTPs and therapy noncompliance in hypertensive patients and should be performed continuously at primary care unit and at home for the benefits of the patients.
